Managing an enlarged pores

What is a pore, the ugly hole on face?

A pore is a hole where hair can come out. Except for the area where eyebrows grow, there are tons of invisible hairs on the face. The size of a pore in general is about 0.02~0.05 mm in diameter and there are approximately 20,000 pores on the face.

The pores, I didn’t think I don’t have them when I was young… but how come they became bigger and larger?

The sebum made in the sebum gland around the root of a hair (a hair follicle) is secreted out of the skin through the pores. Thus, as the size and functions of the sebum glands are increased (mainly during adolescent), the pores become bigger. Also once the skin loses its resilience, the pores strength to hold gets decreased as well. For this reason, pores look large and big.

Process of enlarging the pores
1. Excessive secretion of sebum Creation of sebum in the sebum gland -> Enlarging the pores to discharge the sebum -> Occurrence of various skin problems as the discharged sebum is oxidized with dust and sweat, for instance the build up of black heads.

2. The dry skin that needs moistureExcessive generation of keratin on the dry skin -> Once the keratin fills the pores with dust and sweat, it then subsides, thus causing the pores enlargement and inflammation

3. The aging of the skin The aged skin that is dry and less resilient -> Skin becomes droopy as its strength to hold a pore decreases, thus pores are more remarkable.

4. The waste left in the pores Waste from foreign substances such as makeup, fine dust and skin care products are left in pores. Such materials cause the pores to enlarge.

5. Excessive exposure to sunlight
It enlarges pores since the ultraviolet rays from the sun stimulate them.

The life long treatment for pores, why is there such a need?

The enlarged pores are not to good look at, but more importantly, they increase the risk of germ infection and waste accumulation that may result to overall damage of the skin.

The treatments for pores are through clean skin management

The ideal way of cleansing waste on the skin
The sebum and various wastes in the pores forms a lump that makes the pores look big. For this reason, it is important to clean the pores.

1. Remove the dirt on the face by using cleaning products.
2. Wash the face thoroughly with the foam created by the product using the tips of the fingers.
3. Rinse off with warm water several times and gently tap, not rub, your face to dry.

The massage and mask for the removal of sebum and healthy skin
1. After washing the face, stimulating the face thoroughly with a massage cream like massaging it helps remove remaining waste in the skin. It also enhances the blood circulation so that the skin gets healthier.

2. Wet a clean towel and slightly warm up the towel in a microwave oven for 2-3 minutes. You may also wet a towel with hot water, which can be equivalent to a steam towel. Cover the face with the towel to open up the pores of the face. A towel that is too hot may burn the skin so be aware of the appropriate towel temperature.

3. To moisturize and provide nutrition to the skin, apply essence, moisturizer, nutrition facial mask or sebum removal mask for further skin treatment.

* What is the main purpose of pore treatment products?
The ingredients of pore treatment products suppress the various problems in the pores. Problems such as excessive sebum secretion, lack of astringent, skin ailments and so on, the products help fight bacteria before it causes the pores to enlarge. If a skin type has a lot of sebum, the pores easily expands and such products effectively prevent the swelling of the pores.

I have a jutting chin

A chin whose lower chin is projected forward than the upper chin is called a jutting chin. Not only the level of crossbite is severe but also lower teeth are much projected forward than the upper teeth. A long chin without the crossbite is not called a jutting chin but a long chin or a long face. In Korea, there are many people with a jutting chin, but for Westerners, the lower chin is smaller in many cases. There is no physical difficulty due to a jutting chin. The major purpose of the treatment is to balance the overall face by correcting the shape of face to a state with harmonized face bone.

A jutting chin is inherited, the excessive growth of lower chin

The reason of having a jutting chin is the excessive growth of the lower chin compared to normal growth. The size or shape of a chin can be inherited if there is a relative whose chin is jutted, there is a possibility. Also, if pushing the lower chin forward becomes a habit since childhood or an abnormal operation of muscles that are connecting the upper and lower chins may cause a jutting chin.

The perverse impression is because the lower teeth are projected forward than the upper teeth

The occlusion state of normal teeth is that the upper teeth slightly covers the lower teeth as it is projected forward than the lower teeth, but in a jutting chin the relationship of teeth is opposite to the normal occlusion. In other words, the lower teeth are projected forward than the upper teeth. It is tested by seeing the occlusion state of closing lips naturally. If one often hears ‘You are like a witch in the Snow-white,’ a jutting chin is suspected. If it is not serious, some would hear having a stubborn or strong impression.

The treatment is a combination of orthodontics and surgery

For children who are in the growing phase, an orthodontic device to suppress the growth of the lower chin can be used. However in general, the growth lasts up to 16 to 18 years old, thus, wearing an orthodontic device until such age is hard for children. Once growth is finished, orthodontics and the jawbone surgery can be done to get a fruitful result in aesthetic as well as orthodontic aspect.

The process of the jawbone surgery

* 1st step - the orthodontics before surgery
The orthodontics before the surgery is a preparatory step for the jawbone surgery that prepares for the upper, lower, back teeth to fit well after the surgery. In general, it takes about 6 months to 1 year.

* 2nd step - the surgery
for the jawboneIt takes about 3~7 days of hospitalization.

* 3rd step - the orthodontics after surgery
Finally, the orthodontics after the surgery is a step to arrange the newly moved jawbone and the teeth, which is more like a step for a touch of detailing. It is necessary to consult with an orthodontist for the orthodontics and a cosmetic surgeon who is an expert in facial bone surgery for the surgical part. It is recommended to find a specialized clinic that provides a medical treatment of both orthodontics and facial bone surgery through specialized cosmetic surgeon.

The males who dream of beauty are increasing. The male cosmetic surgery

About 20 years ago, people visited a plastic surgeon discreetly and carefully. However, today, having a cosmetic/plastic surgery is no longer a secret and a thing to hide. Moreover, it is regarded as a choice to enhance one’s value.

The desire of men to be beautiful is increasing. The male cosmetic surgery

The perception of people has changed gradually, and it became a popular product for both male and female. According to a research made in an organization, about 50% who visited the web-page of cosmetic surgeries were males. Such result reflected the interest of males in cosmetic surgery, and in fact 10~20% who receives treatment from a cosmetic surgery are males.

Cosmetic surgery for males before was to restore one’s damaged look after a terrible accident or to form an area that is now functioning well due to an innate or posterior factor. However, today cosmetic surgery for male is to soften one’s impression or to look more handsome.Such trend can be confirmed from the news of the President Noh who received double eyelid cosmetic surgery. There has been a favorable criticism that his eyes got bigger and he looked younger. Maybe from this moment, the combination of male and a cosmetic surgery, which is somewhat awkward, may have changed to a familiar one.

We do cosmetic surgery for this reason. The male cosmetic surgery

The areas that are frequently treated nowadays are fat removal under the eyes, breasts, hair implant, fat removal, nose, chin, and so on.

A man with a good impression. I will be the man!
The two major factors of male cosmetic surgery that have changed along with the trend are first having a surgery to get a good impression. Wanting to make a good impression and enhance one’s own value through a cosmetic surgery are some of the mindsets of males that undergo cosmetic surgery in 20’s~30’s.

A man with a good body shape. I will be the man!
Another thing on the trend of cosmetic surgery is its shift from face to body. Particularly, cosmetic surgery for hips is increasing along with the breast cosmetic surgery, and such trend reflects the psychology of males who want to be ‘a man with good body’ rather than ‘a man with handsome face.’

It is effective but there are still side effects to consider

Likewise, a moderate level of cosmetic surgery can increase one’s self-esteem as well as his value so that it works as a way to get a better job and meet good people. However, there may be side effect besides such merits; thus, it is recommended to consult with a medical professional for more careful decision.

In addition, the cosmetic surgery among males in their 20’s is showing the highest frequency today but that of 10’s is gradually increasing as well. The cosmetic surgery of 10’s whose bone structure is yet to be complete would cause unexpected negative results.

Hot Flushes, which reveals one’s inside story. Are you still showing this?

Hot flushes that drinks alcohol with face?

In cold winter, there is something that Jack Frost stamps on the face of people in the street one by one, and that is the reddened cheek and nose. There are relatively many blood vessels on face compared to the other areas of the body. Unlike the lips that look red because of many blood vessels and the thin layer of the skin, a nose and cheek can also look red due to some reasons as the blood vessels get extended and the blood rushes such areas. Especially during winter, there are many factors that naturally make the blood vessels extend based on our living condition, such as staying indoors with a great temperature difference from outside, frequently having hot beverage or food, and so on. Thus, a reddish face is often seen especially during the winter.

Nonetheless, there are certain people who often hear, “Your face is so red. Did you drink a lot?” or “Are you embarrassed?” Such people are the ones with hot flushes. Hot flushes make one’s face burn with minor emotional or temperature change or without a particular reason. In fact hot flushes do not call an immediate medical treatment, but often cause misunderstanding for people with hot flushes. As such, many people are seriously concerned about this bothering symptom.

The cause of hot flushes - the autonomic nervous system, emotional change, systemic diseases

The hot flushes caused by an embarrassing emotional fluctuation

Being reddish on face or neck when one is excited, angry, embarrassed, or shy is related with the autonomic nervous system. The sympathetic nerve that forms the autonomic nervous system ? uncontrollable with one’s will ? catches a signal and speeds up the heart beat when one faces a situation mentioned above. During such time, hot flushes happen as the blood vessels on face are extended and blood rushes into the face. Further, such symptom is more intense when one can not cope with a psychological or stressful situation.

The hot flushes caused by the heat production to adjust oneself in a low temperature

When the weather gets cold, our body produces heat to adapt to this change, and because of this our body parts can be reddened from ears whose skin layer is thinnest, and then the nose and cheek. As the blood vessels are extended and blood rushes, the face can be reddened, but if there is a problem in the blood vessels, such symptom would last regardless of temperature. In addition, if our body is exposed to cold for a log period of time, teeth and the body are trembled to produce heat and to reduce the amount of radiating heat from our body, our body gets crouched or the blood vessels can be contracted. During such time, the lips or face may get either blue or white as the blood vessels are contracted.

The hot flushes caused by eating hot or spicy food or drinking alcohol

When one cannot neutralize alcohol well, it makes his/her face red. It is the sign of ‘not a body for alcohol’ so to speak, because lacks the decomposing enzyme for alcohol. Hot and spicy food, walnuts, cheese, and chocolates stimulates the autonomic nervous system, thus, may cause hot flushes.

The hot flushes caused by taking a vasodilator

When taking medicines for hypertension (Nifedipine, Verapamil), heart disease (Nitro Glycerin), hyperlipidemia, tuberculosis (Rifampin),, hot flushes may occur more frequently, and upon the termination of the medication the symptom will disappear.

The hot flushes caused by exposure to a hot temperature, such as sauna or fomenting room

High temperature makes the blood vessels in the body enlarge to discharge heat. Especially, the blood vessel around cheek on face that has a lot of vessels expands more to make face red.

The total hot flushes caused by a physical problem or disease

Carcinoid syndrome (a tumor of Carcinoid cell that secretes various hormones), diabetes, Mastocytosis, a tumor in the pancreas, kidney, or adrenal, the cold sensitivities may cause the hot flushes, and thus, it requires an internal diagnosis. (source: The Rare Diseases Helpline, National Institutes of Health, Korea Center for Disease Control and Prevention)

The hot flushes during menopause caused by hormonal change

The hot flushes that 75% of women in the menopausal period are experiencing is because the body temperature rises. This symptom disappears within 2 years for 80% of them, but some reported that the symptoms lasted about 5 years. When weather is hot or one is under stress, in other words, external factors may worsen the symptom, and it also triggers a trouble in sleeping and fatigue as major causes that torment the women in the menopausal stage

The hot flushes caused by other factors

- Being exposed to sunlight (ultraviolet rays) often damages resilience fiber that is wrapping the blood vessels and causes hot flushes as the blood vessels gets transparent. - When there is atopic problem or pimples on skin, hot flushes may occur as an aftereffect of the blood vessel expansion.

How is the hot flushes treated?

After determining the major causes of hot flushes, an appropriate treatment will be given. When the symptom is due to an internal disease, it has to be treated before taking care of hot flushes.

If one gets emotional easily even to a trivial thing...

For a person who has a hot temper and often experiences frequent emotional swings or is so timid that it is hard to hide what he/she feels on his face, it is recommended to consult with a psychiatrist to stabilize the states of emotion. Certain medication that prevents excessive stimulation of the autonomic nerves may be

Regular exercise and peace in heart are important for the women in the menopause

Continuous light exercise like jogging can help one can stabilize mind, and by having the estrogen-rich foods such as beans and tofu, one can overcome the menopausal disorder. If the symptom is severe, the hormone therapy can be accompanied and it is recommended to consult with a medical professional.

Moderation in drinking is the best remedy for the face that gets red when drinking

Worrying about the reddening face after drinking although one does not have a sufficient ability to decompose alcohol is same as the mind that wish a car to go with water. One’s drinking capacity has nothing to do with learning, thus, never ignore such signal the body is sending and try to drink in moderation.

Taking vitamin K that hardens skin and blood vessels

It is effective to take vitamin K that hardens the drooped capillary vessels and skin or apply vitamin-rich cream. Foods rich in vitamin K are seaweeds like brown seaweed, laver, or kelp and green leafy vegetables such as lettuce and spinach.

Other medication

The ointment with antibiotic substances has side effects as it enlarge blood vessels, thus, nonsteroidal drugs are recently used. Such medication has to last minimum of 6 months, and further surgical intervention may be decided to monitor the result of using the ointment.

The blood vessel laser treatment that destroys only the expanded blood vessels

If there is a pigmentation on a certain part on the face because of the severe blood expansion or pigmentation that causes hot flushes or one experience a serious skin trouble as an aftereffect such as a tumor, a more active medical intervention may be considered.

By using homodyne laser that is applicable only to blood vessels, the major cause of hot flushes can be removed as the laser selectively destroys the enlarged blood vessels. Depending on cases, the laser treatment is performed 3-5 times in 4 weeks interval, and there may be a light level of flushing and swelling (about 1-2 days, may last 2 weeks maximum). Recently, the multi-wavelength laser whose treatment intensity is not stronger than the homodyne laser is proven to destroy both short and long blood vessels simultaneously and is used to minimize aftereffects like bruise and swelling.

Further, the nerve block treatment that prevents the expansion of blood vessels by blocking the sympathetic nerve can be applied.

Ways to prevent and caring for hot flushes

1. Avoid exposure to hot temperature. A period of time for bath or sauna should be short and try not to use a fomentation room.

2. Apply ultraviolet ray (major cause of destroying resilience fiber and the expansion of blood vessels) block cream and moisturizing cream.

3. Wear thick clothes to keep the whole body warm when cold and try to wear a mask.

4. When entering from a cold place to a warm area, wrap both cheeks and massage them.

5. Avoid too hot and spicy food. Try no to have walnuts, cheese, chocolate, and alcohol.

6. Do not apply any ointment on the face. If an ointment is steroidal drug, it would worsen the symptom.
